Leonid Zakharin(Israel) Alexander Myasnikov: a doctor in America and Russia.

Пятница, 10 Ноября 2017 г. 10:53 + в цитатник

יום הולדת 90 לזמרת סבאח

Александр Мясников: врач в Америке и России

Alexander Myasnikov, MD, PhD

Chief medical officer, Family Physician, Internist, Cardiologist, Anti-aging Specialist.

Dr. Alexander Myasnikov is a representative of the third generation Moscow medical tradition, tradition, that enriched by his American training.

Alexander graduated from the State University of New York and Brookdale Hospital Medical Center. Prior to this, he graduated from the Pirogov Moscow Medical Institute in 1976 and finished his Cardiology Fellowship at the National Cardiology Center of Russia. Early in his career, Dr. Myasnikov had been practicing for four years as an internist, cardiologist and general practitioner in Africa and France with the International Organization for Migration before he returned to Russia.

In 1996, he was certified by the Educational Commission for Foreign Medical Graduates, and began his training in the USA. Then he became certified by the American Board of Internal Medicine, as Cardiologist. He is a member of the American Medical Association, the American College of Physicians, and the American Academy of Anti-Aging Medicine. Alexander is the former Medical Director of the American Medical Center.

Dr. Myasnikov opened the American Medical Center here in Moscow in 2002.

Now Alexander Myasnikov is the Medical Director of Kremlin Clinic in Moscow.

This has already been published.
Nevertheless, the advice is practical and not too burdensome.

The medical dynasty of Myasnikovs is one of the most famous not only in
Russia, but also in the world.

According to the textbooks of one of the representatives of this dynasty, the founder of
Soviet cardiology, Academician A.L. Myasnikov,
still students of all medical schools are engaged.

Doctor Aleksandr Leonidovich Myasnikov worked for many years abroad.
He started with Africa, then he worked in France and, finally, after passing the
American licensing exams, he got a seat in the residency of one
of the hospitals in New York. Now he works in Russia.

Alexander Myasnikov: a doctor in America and Russia.

In America, the life of a doctor is so crowded that it can be said that it simply does not exist.

The American doctor comes to work at 5 am, and leaves at 10 pm.
Every third day he remains on night duty, no time off
for this is not allowed, and the next day he at 5 am again at work.

Add to this 2 hours for the journey and 6 hours for sleep and you will understand that he has
no choice for personal life, for children, or for the wife of time

Will pass 10, 20, 30 years - nothing will change: there will be all the same
hospital corridor, detours and you, spinning in all this, like a squirrel in a
wheel.

At some point I realized that this is not really mine.

Medicine in Russia and medicine in America are two completely different things.

The West is moving forward, and we were stuck somewhere in the mid-1970s
, when medicine was largely
empirical.

The activity of a doctor in the West is strictly regulated.

On all there is a scheme of treatment, which he must adhere to.

If, in the treatment of a patient, a doctor of five steps does the first, second,
third, then fifth, and then returns to the fourth and thereby
saves the patient, he is in very great trouble, up to
prosecution.

If the doctor will treat strictly according to the scheme: one, two, three, four,
five and after that the patient will die, no one will say a word to him, because he
did everything according to the rules!

To Russians who are not used to this approach, it may seem
absurd, but believe me: deviation from the standards sometimes
can really save one, two, three people, whereas a mass
non-compliance with the adopted algorithms can lead to very serious
consequences.

By the way, in the first year of my stay in the US hospital they wanted
to fire me with the wording: "he makes diagnoses based on intuition, and not on a
clear analysis of all medical and diagnostic data."

The standards of treatment provide the patient with protection from an incompetent physician.

We have a lot of times how?

A person comes to a polyclinic, they prescribe a medicine for him.

Some time passes, he gets to another doctor, and he says:
"Who has this appointed you? We should not take this, but this "... Then,
for some reason, he goes to the third, who says that the two
previous doctors did everything wrong, and appoints a completely different
drug.

That is, the person is completely defenseless before the doctors.

Doctors in New York will never say that you were treated
incorrectly in Detroit In New York, the doctor will look at what you were treated in Detroit, evaluate the
results and, if they are unsatisfactory, relying on the standard,
try other medications, then the third.

If something goes wrong, he will begin to think about why thousands of patients
help, and this, specific, no? And work will begin on
standards, that is, a further diagnostic process.

It is right. It should be so.

God willing, and we'll come to this someday, and then everyone will know that the
best medicine for hypertension is a penny diuretic, from angina pectoris
- aspirin, from inflammations - tetracycline worth 15 rubles, and not
expensive imported preparations.

The system of standards will also save patients from painful and often
useless operations. In Russia, with sinusitis,
punctures are still being done , although it is proved that this procedure does not alleviate the ailment.

Deep vein thrombosis worldwide is treated with blood thinning
medications, but the patient is operated on.

In general, I say this: you can argue long, figuring out where it's good, and where it's
bad. But here are the facts: an American in 80-85 years in a hunt in his
garden, nurse grandsons, travels around the world, and in Russia the graves of the
majority of his peers are children. Unfortunately, they did not
wait for their grandsons.

Alexander Leonidovich Myasnikov about free medicine.

I fight for free medicine, but only for certain categories of citizens.

Now for each patient in the medical institution goes to the system of
compulsory medical insurance (CHI) a certain amount of
money.

And this is absolutely correct. Another thing is that the CHI tariffs do not cover
one-third of the cost of all costs.

Relatively speaking, I spent ten rubles on treating the patient, and on
insurance only three returned to me.
To me such treatment at a loss. Today, for the grandmother, the child, the
able-bodied person with an average income and the oligarch, we get the
same according to the compulsory medical insurance (in Moscow it is 7 thousand rubles a year) financing.

In my opinion, this is wrong.

I suggest an absolutely simple and logical thing. Elderly and children
should receive medical care free of charge. To working - a
differentiated approach. Suppose we brought a person unconscious to
the ambulance - vital manipulations are free of charge, and for everything
else he can pay. Well, let the rich pay the
full program. In my opinion, it will be so fair.

Butcher: how to live longer.

I am a full member of the American Academy of Medicine against
Aging. In America, this is financially profitable and
interesting.
Back in Russia, I lost interest in this direction. I'll
explain why. What is an American who wants to live long?

He does not smoke, watches the level of cholesterol and sugar, maintains a
normal weight, runs and swims, with a preventive purpose visits a
doctor not one, but several times a year.

And take our pensioner. Pressure 160/100, cholesterol for eight
mmol / l, sugar for seven, flabby, smoking and at the same time declares:

"I do not want to grow old" ...
We, to my deep regret, are ailing nation. For us, the main thing now
is to stop destroying ourselves.

Doctor about medicines.

Open almost any magazine, turn on the radio or TV and immediately
learn about a new magic tool that heals everything in the world. This is a
huge business, whose members are trying to make money on our
diseases and weaknesses.

We are very convenient for such businessmen, as they are not ready to
give up the sausage for health reasons , even if we give them an hour of physical
exertion. We are not ready to go to a medical institution for a
preventive examination.

But we want a magic pill that will solve all our problems at once.

In Russia, drugs are taken more than in all countries of the world. But the
world practice shows that enough 50 titles
preparations. We are popular with expensive drugs with unproven
effects, various hepatoprotectors, immunomodulators, probiotics.
All this is a wiring for money.

In the autumn, a lot of advertising of antiviral drugs appears: "Drink and not
get sick with the flu!"

Most of them are useless. In order not to get sick with the flu -
do the vaccination, lead a healthy lifestyle, temper,
exercise, eat right and do not
stuff yourself with all the chemistry!

As a rule, the usual infection passes itself within one-two
weeks. You just have to lie down, drink a lot and wait.

All will pass or take place itself without medicines and especially without antibiotics. They
are not affected by a virus infection at all!

What does not love the heart?

Our heart first of all does not like tobacco. This is enemy number 1. Secondly, a
sedentary lifestyle. Among the risk factors for the development
of cardiovascular pathologies,
inadequate intake of fruits and vegetables is not the last place

Every day we need to eat, do not just faint, half a kilo of
fruit and a pound of vegetables.

The heart does not like unhealthy fats: sausage, ice cream.

Sunflower, olive or any other vegetable oil is useful.

The heart does not tolerate a rise in pressure, which means that we are fighting hypertension and,
first of all, we reduce salt intake.

By the way, we are the only country in the world that consumes 12 grams of salt per day
per capita at a rate for a healthy person - 5 g.

I repeat, for a healthy person, and for hypertension, for a person who is
over 50 years old, the norm should be 1.5- 2 g.

To all my patients, suffering from this or that heart disease, I
do not get tired to repeat: "Do not forget for a minute that a
man with an ax is following you , so be extremely careful.

Do not run after the bus - wait for the next one.

Do you want to sunbathe in a hot day?
A hundred times think about whether the game is worth it.

A slight dehydration can increase the viscosity of the blood and lead to a heart attack.

Do not be nervous over trifles. And not over trifles, either.

Even a little stress can change the functional state of the
platelets, they will begin to stick together with each other and will clog the flow of blood
in the heart.

Remember the peasant with the ax behind you. One God knows
when he will knock you, but it can happen at any second. So
think before you take this or that step.

Dr. Myasnikov advises:

First.

Move, move and move again. Even if you really want
to lie down, still overcome your laziness and move, move,
move!

The second.

Never feel sorry for yourself and do not depress you. Stress and
depression "eat" us. Many are worried about tomorrow. Do not do
this. The future has not yet come, so there is nothing to think about it.

Be sure to follow the principles of healthy eating.

Expression: "With a fork and a spoon we're digging our own grave" was plagued, probably by
everyone in the ears, and yet it is so. Categorically refuse
smoking, use alcohol as little as possible.

In the diet, focus on vegetables, fruits, fish.

If someone does not like fish, you can take vitamin D, which
is sold as omega-3 - cod-liver oil.

It is very useful to eat a day at least one clove of garlic, which
contains a large number of flavonoids, protecting the
heart.

Many flavonoids are also found in black chocolate. One cube of
dark chocolate a day can be considered an acceptable substitute for garlic.

Contain the growth of cholesterol and even lower its nuts, especially
almonds, so I believe that 70 grams of nuts per day for the core are
mandatory.

If you really want to eat something
delicious (and we know that everything is delicious - it's harmful - eat it, but just a little, just a
 small piece.)

And let it not be a sausage, I believe, should be
banned as well as advertising tobacco and alcohol, because it is no
less harmful.
