Season 4, Episode 19 - My Best Laid Plans

Kylie and J.D. have been dating for a month, but she still won't sleep with him, leaving J.D. very frustrated. Turk and Carla are still going through their wedding growing pains. Turk complains that Carla always blows things out of proportion. Speaking of relationships, Dr. Cox and Janitor have a bet that Janitor and Elliot will end up together. Cox bets the Janitor his Porsche; Janitor wagers his van.

To further complicate J.D.'s love life, Molly Clock (Heather Graham) returns to the hospital. She's only back to see one of her former psychiatric patients. J.D. gets another shock when he realizes that Turk is talking on the phone to his old college girlfriend, Rosanna. J.D. takes the phone from Turk and throws it out the window. The phone makes it way back to Carla, who asks Turk why he has three missed calls from his ex-girlfriend Rosanna. He accuses her of making a mountain out of a molehill, so Carla retreats. Re Kylie, Turk advises J.D. to have some fun with Molly and then when she leaves he can re-focus on Kylie. Sure enough, Molly asks J.D. if he wants to go out for a beer. He happens to be on the phone with Kylie when she asks, but J.D. accepts Molly's invitation anyway. And Janitor seems to be winning his bet with Dr. Cox after Elliot jumps into his arms and declares him her man. Sure enough, Cox gives Janitor the keys to his Porsche. But, Janitor has his own trick up his sleeve when he tells Elliot that Cox needs to see them on an actual dinner date.

Carla realizes that she can't stand Turk talking to his ex, so she demands that he end the relationship completely. He agrees, so she thinks. When Elliot finds out that the Janitor lied to her their date, she's upset, but when he tells her that she's the only one that treats him like a real person, she calms down.

J.D.'s night doesn't end as well. When he stops by Kylie's apartment, he mentions that he had the opportunity to be with another girl, but he held off because of his feelings for Kylie. She gets angry and throws him out. He tries calling her the next day, but she doesn't respond. When Carla finds out that Turk's ex just now found out that he's married, she's furious and storms out, leaving both J.D. and Turk without their women.

J.D. encounters relationship problems with Kylie (Chrystee Pharris) and Carla and Turk. Kylie won't let him spend the night, even though they've been making out like crazy. Carla and Turk want him to move out because he's getting in the way of their newlywed bliss. Meanwhile, Dr. Cox is trying to outdo his high school buddy Ron Laver (Michael Boatman) who is in town for a visit. Their lives have been parallel in a lot of ways: Cox went to medical school; Laver went to business school. Both men are divorced; the both have 2-year-old sons.

He's not happy about it, but, J.D. offers to move out for a week. If Turk/Carla don't miss him he'll find a place of his own. He asks Elliot but she says her landlord told her J.D. couldn't come back to her apartment after she wailed so loudly when he broke her heart. He then tries Kylie. That doesn't work because she's still not ready to sleep with him, which makes them both uncomfortable. He ends up staying with the Todd.

Instead of going to dinner, Ron Laver brings his 2-year-old son Nathan with him to Cox's house. Even there the two men continue their pissing contest, comparing whose son is more talented, taller, smarter, etc. Carla and Turk invited Elliot over. They needed someone to talk to after they finished having sex in every part of the house. They eventually kick Elliot out after she declines to gossip about their co-workers. Turk and Carla finally admit that they miss J.D., who's not having a good time staying with Todd. And after Laver's son Nathan builds a perfectly symmetrical, color-coordinated tower from building blocks, Cox thinks he may have autism.

Cox spends some time alone with Nathan, which reconfirms his initial suspicions. Ron doesn't take the news well at first, but later comes to realize that he needs to be strong for his son. Cox gives him the name of a doctor who lives near Ron that can treat Nathan.

J.D. stops by Turk and Carla's, hoping they'll ask him back. Instead, they say nothing. The Janitor offers him a place to stay, and so does Ted. He stops by Doug's house, but it turns out he moved away. The house is now occupied by Ed McMahon (Ed McMahon). He stops by Kylie's house and she finally asks him to spend the night. Just as they are about to head to the bedroom, Turk and Carla storm in. They can't live without J.D. and they want him back.

J.D. is giving Kylie a sitcom quiz. She passes with flying colors. Later, she and J.D. and Turk/Carla watch "Sanford & Son," one of J.D.'s and Turk's all-time favorite sitcoms. Carla is a little jealous that J.D. and Kylie are so loving and affectionate with each other while she and Turk are just an old married couple now. A news report comes on after "Sanford & Son" warning about tainted meat. The next day, the hospital is overrun with people thinking they have an E. Coli infection from the bad meat. J.D. comes across one patient with the name Charles James. J.D. asks him if he's the same Charles James who was a writer on "Cheers." It turns out that he is. J.D. is beside himself with excitement. When J.D. hears Mr. James' hacking cough he immediately orders a chest x-ray. While they wait for radiology to have a space open, J.D. continues to bother Mr. James with "Cheers" questions. The x-rays show that James has metastatic lung cancer. J.D. wishes that life was more like a sitcom.

Later, Kelso tells Cox that he has to cut $27,000 from the cafeteria budget. Cox boasts that he can do it without having to fire anyone. But after examining the papers, he realizes that someone will need to be fired. He identifies Kenny (Clay Aiken) since he was the most recent hire. Kenny of course, is completely enthusiastic about his job, which makes firing him all the more painful for Cox.

In J.D.'s sitcom fantasy, we're in front of a live studio audience. In the fantasy, Mr. James knows that he has lung cancer. Since he's probably not going to make it, Elliot arranges for Mr. James to take a balloon ride, have lunch by the beach and go to his first carnival.

Meanwhile, Dr. Cox has to tell Kenny, that he can't stay on at the hospital as a cafeteria worker because they can't afford to pay his salary. Cox gets the idea that they can have Kenny perform in the hospital talent show, where the grand prize is the same, exact amount of his salary - $26,372.

Elliot and Carla perform a scene from "Romeo and Juliet" for the talent show, complete with elaborate costumes. Carla is Romeo and Elliot as Juliet, recites her lines in German. The Janitor performs a stand-up routine, but the audience doesn't find it funny. Turk and J.D.'s stunt is as a giant doctor holding a live chicken. Suddenly, Kenny comes in, looking very handsome (unlike his earlier nerdy look). He sings an amazing rendition of Stevie Wonder's "Isn't She Lovely." Kelso declares Kenny the winner of the talent show. Then, a few minutes later, Elliot realizes that she has been reading the wrong chart for Mr. James. She actually has James Charles' chart. He's the one that has cancer. Suddenly, Mr. James passes out. Colin Hay ("Men at Work") shows up to sing the "Cheers" theme song. The fantasy scenes then switch back to reality.

In reality, Kenny ends up getting fired, Elliot and J.D. work on reviving Mr. James and Turk/Carla try to focus on improving their relationship with each other.

J.D. is excited about his first date with Kylie (Chrystee Pharris). On the way to dinner, J.D. runs over a possum, which dies, and a homeless man's hand. They end up taking Steve the homeless guy to the hospital. J.D. gets a surprise when Danni (Tara Reid) shows up. He hasn't seen her since their last break-up and J.D. doesn't want Kylie to be exposed to Danni's toxicity. J.D. is handed the chart of a patient who may have SARS-related symptoms. Suddenly, the hospital has to be shut down for quarantine while they investigate his condition. It's policy that the hospital be locked down until the labs confirm that it is or isn't SARS.

Since she's stuck with J.D., Kylie mentions off hand that she wishes she could be his assistant. J.D. makes Lonnie take off his shirt so that Kylie can wear it. Everyone else tries to find something to do. Carla has Turk changes the dressings on a patient since there's no surgery for him to do. Cox tries to avoid Danni. Dr. Kelso bets the Janitor that he can't catapult a cotton ball into a container with a tongue depressor. The Janitor misses on the first try, so Kelso tells him double or nothing. He keeps missing, racking up a total of $600. The final total reaches $700, none of which the Janitor has. Dr. Kelso tells him that if he gets him a piece of Nurse Laverne Roberts' (Aloma Wright) face cake, he'll forgive the debt. When Nurse Roberts notices a piece missing, Dr. Kelso points to Janitor. Still bored, J.D. has Steve the homeless guy pretend to have a heart attack so J.D. can go through the motions of trying to save him so Kylie will think he's a great doctor. J.D. originally gave him $20, but Steve demands $50 to make it look good. He feels guilty about lying to Kylie since her last boyfriend lied to her, but he gets over it. Turk gets upset when he finds out that Carla and Dr. Cox dated briefly.

Finally, they get the OK that the patient doesn't have SARS. Dr. Kelso is just about to leave the hospital when Laverne grabs his hand. She spotted some icing on his thumb, icing from her face cake. She tastes it and confirms that it is indeed her icing. Janitor ratted Kelso out. The Janitor tries for one more shot, double or nothing. It goes in!

J.D. finally admits to Kylie that he's been lying to her. She takes it in stride and they end the evening with a kiss.

Season 4, Episode 14 - My Lucky Charm

Billy (Colin Farrell) has brought a guy to the hospital because he was knocked unconscious after a bar fight. At first J.D. and Turk think the guy is Billy's brother because that's what Nurse Roberts told them. They later find out that it's not Billy's brother, but Billy doesn't know his name or any other  (150x225, 9Kb)information about him. Billy admits that he's the guy who knocked him out.

Billy is a free spirit who embraces life. He tries to impart that same spirit to J.D. and Turk. The way they act together, Billy initially thinks that they are closer than just friends, but they quickly explain that they are just good buddies. Billy's philosophy of experiencing new things extends to making out with Elliot. He almost kisses Carla after he tells her that her hair is curly, but Turk steps in just as she's about to kiss Billy.

Because Billy is paying them so much attention, Carla and Elliot each become jealous of the other. It escalates into them fighting each other in the cafeteria.

Meanwhile, Carla and Elliot keep breaking their plans to hang out together and Dr. Cox has a vasectomy but doesn't tell Jordan. She thinks he went fishing with his buddies. His secret is revealed when Dr. Kelso recognizes Cox's limp from when he had his vasectomy. Cox is surprised when Jordan doesn't get angry at him. She later tells him that she wasn't surprised by the decision and she supports him. Cox surprises himself by getting the vasectomy reversed. He surprises himself by telling Jordan that he wants to communicate more openly with her and wants to try and be a real family. In the spirit of open communication, Jordan tells him that she loves him and doesn't want any more kids and wants him to get the vasectomy that he just reversed. Cox is happy.

After J.D. and Turk find out that Billy is the reason their patient is in the hospital, they can't decide whether to report him to the police for assault. It turns out that J.D. had already called the police, but he used Turk's name. The police take Billy away in handcuffs. He returns shortly to see the patient that he knocked out, who is now conscious. Billy chides J.D. and Turk for not talking to him directly about what happened.

After Billy leaves, everyone realizes that they need to appreciate their lives and their relationships more. Elliot and Carla decide to re-establish their friendship, Turk and J.D. decide to enjoy life more and not be caught up in work and Jordan is by Cox's side when he goes in to have the final vasectomy.

J.D. accompanies Turk and Carla to a nightclub. It's packed with black people and J.D. and Turk are the least hip people there. J.D. is immediately smitten with the gorgeous woman behind the bar. He asks Kylie (Chrystee Pharris) if she can make him an appletini. It turns out that that is her favorite drink. It also turns out that she needs the services of a doctor, so it's a great coincidence that J.D. is a doctor. Kylie shows up at the hospital the next day. But instead needing treatment herself, she brings her boyfriend, James (Marc D. Wilson). J.D. is crushed.

While taking James' medical history, J.D. uses it as an opportunity to dig for information. He finds out that James and Kylie don't live together and they're not engaged. It turns out that James has gonorrhea. Even though he and Kylie have been dating for three months, they haven't slept together. She wanted to wait, so he's sleeping with a few other women until she gives in. J.D. can't believe that James is doing this. But more importantly, J.D. is bound by doctor-patient confidentiality, so he can't tell Kylie anything.

J.D. decides to tell Kylie, even though it violates his Hippocratic Oath. But, he changes his mind at the last minute. Kylie and James drive off together in his nice car. Things turn around when Kylie shows up at the hospital the next day. She says that her friend and fellow bartender Tamyra has similar symptoms like James. J.D. then blurts out that it's gonorrhea. He explains that he couldn't tell her because of the rules. She then says that James wasn't her boyfriend.

Elliot's patient, Mr. Cheng, is in a coma because of renal failure caused by malaria. Since Mr. Cheng is older and his organs are failing, Dr. Kelso doesn't want to try any preventative measures to prolong his life. Elliot isn't sure what to do. Later, she gets up the courage to stand up to Dr. Kelso. She announces that she wants to do an exchange transfusion. Kelso vetoes her since Mr. Cheng's chances for survival are slim. But when they meet with the family, Elliot decides to tell the family that they might have a chance if they do the exchange fusion. The procedure brings Mr. Cheng out of his coma briefly, but then he dies anyway.

J.D. and Elliot have managed to handle their co-chief duties like a well-run machine. J.D.'s strengths are Elliot's weaknesses and vice versa. Elliot is stumped as to why her patient, Mrs. Kasuba has abdominal pains. J.D. offers to try and find a diagnosis since that's his strong suit. Sure enough, he determines that she has acute intermittent porphyria. He tells Nurse Roberts (Aloma Wright) to get the patient on IV dextrose and hematin. She tells him that Dr. Reid already ordered that same treatment. J.D. realizes that Elliot discovered the condition before he did. He catches up with Elliot in the patient's room. When Dr. Cox comes in to congratulate Elliot, she gives all of the credit to J.D. That's when he realizes that she is taking pity on him since he botched a code on one of his patients.

J.D. later acknowledges that he's having a hard time dealing with the fact that Elliot has become a much better doctor. He even tells her that she's the one that should be struggling, not him. She tells him that the secret to her success is to breathe when things get crazy. When he finds himself in a situation where a patient is crashing, he uses the breathing technique, which allows him to think more clearly and he is better able to handle the situation.

After he operates on a patient who is about to loose a foot because of diabetes, Turk realizes that he needs to take his disease more seriously. Janitor almost reveals to Elliot that he has a crush on her, but decides to keep it to himself.

Mr. Milligan (Matthew Kaminsky) is J.D. and Cox's new patient who is diagnosed with a serious case of mononucleosis. Since it's almost the holidays, he doesn't want to be stuck in the hospital, particularly since he has his 11-year-old son Tyler with him. J.D. promises him that he'll be out before Christmas. Cox immediately pulls him from the room to scold him for promising something to a patient that he may not be able to deliver. Sure enough, Carla comes out of the room to tell them that Mr. Milligan is unconscious.

Elliot takes care of Tyler, even though little kids freak her out. Carla mentions that Milligan doesn't have any insurance so Kelso wants him transferred to County hospital when he is stabilized. In the meantime, Elliot gives Tyler $1 to go to the vending machine for a Yoo-Hoo and he disappears. Cox and J.D. still can't find a cause for Milligan's illness. They remember what Milligan said about he and Tyler wrestling and how Tyler dropped him with a pretty powerful kick. J.D. suggests that maybe his spleen ruptured.

Kelso has a change of heart about sending Milligan to another hospital when he and Tyler have a nice chat about baseball. Elliot finds him with Kelso and takes him back to his father. She has him watch the operation from the viewing gallery. This upsets Turk because he feels more pressure when someone's watching.

The operation is a success. In post-op, J.D. snaps into action, ordering everyone around. On Carla's advice, Cox steps back and lets J.D. handle everything.

Milligan and Tyler leave the hospital, still in time to celebrate Christmas.

Throughout the episode, all of the doctors reminisce about their "best moment in medicine."

The scariest malpractice attorney in the city, Neena Broderick (Julianna Margulies) comes to Sacred Heart to wreak havoc. Everyone is afraid of her steely personality except for Dr. Cox, who seems to thrive on it. Things go from bad to worse when Cox and J.D. realize that their patient, Mr. Steel, is Neena's father. Surprisingly, Neena asks J.D. to fill her in on what's happening with her father. Over coffee, J.D. realizes that Neena puts on the tough-girl act for her job but she really is sweet. Dr. Cox doesn't want to implant a defibrillator in Mr. Steel's chest because of the risks and he's not in a life/death situation, but Neena convinces J.D. to try and persuade him.

J.D. realizes that Neena is just playing him so that he'll do the procedure on her father. He later realizes that he has to do the right thing, so he tells Dr. Cox to do the procedure. Everything turns out fine. Even better, J.D. and Neena hook up at his apartment.

Patient Mr. Norman (Richard Kind), has returned for the 27th time complaining of yet another ailment. He's upset because his tennis serve hasn't bounced back since his shoulder surgery six weeks ago. Turk tries to tell him that it will take a while for the shoulder to heal. Meanwhile, Norman continues to stalk Turk, even following him home. Turk tells him that he's taken out a restraining order on him. Mr. Norman turns the tables on him by suing him, with Neena as his attorney.

Elliot tries to find a way to tell Doug that he's not cut out to be a doctor. But, it's later revealed that Doug would make a great pathologist. He seems to have an uncanny knack for figuring out how people died.

Season 4, Episode 10 - My Female Trouble

Turk and Carla are upset with J.D. now that he is dating Neena (Julianna Margulies), who is suing Turk for malpractice on behalf of Mr. Korman (Richard Kind). Korman is suing because the shoulder surgery Turk performed ruined his tennis serve. On Turk and Carla's urging, J.D. tries to break up with Neena, but she won't let him. To make matters worse, Kelso orders Ted the lawyer to settle Korman's case, giving another victory to Neena.

Fed up with all the nonsense, Carla brings Turk and Mr. Korman together. Korman admits that tennis is his only way of connecting with people, and since his serve has been affected, he's lonely. He agrees to drop the frivolous lawsuit and Dr. Cox is recruited to be his new tennis buddy.

Later, Turk and Carla catch J.D. and Neena having sex in the meeting room. They leave pissed. J.D. gets Jordan to break up with Neena for him. Jordan and Neena are the two meanest women he knows. Jordan and Neena square off like fighters from "Kill Bill." Jordan is the victor. Neena takes Todd as her next victim, while Jordan makes J.D. wear her designer jeans to stretch them out.

Elliot's new patient, Mr. Summers (Jon Polito), is a sexist hospital board member, who uses his obnoxious attitude to get away with anything and everything. When Elliot enters his room, he assumes that she is the nurse instead of the doctor. Realizing that he's going to be difficult to deal with, Elliot decides to play along, posing as a nurse. She relays Dr. Reid's orders to Mr. Summers. But, she's caught in a quandary when Summers asks to meet Reid. Elliot recruits The Janitor to pretend to be her. Things are going relatively smoothly until Cox and Jordan happen to walk by. Cox is disappointed, mainly because Elliot had grown so much in the last few months and she wasn't relying on him as much. She defends herself by saying that she treated Summers the best that she could under the circumstances.

Season 4, Episode 11 - My Unicorn
November 23, 2004
J.D.'s patient, Harrison Marks (John Bennett Perry), needs a new kidney. He doesn't have any family around who could donate a kidney, but he does apparently have an estranged son. J.D. is determined to find this son, since it could be Marks's only hope for the future. J.D. manages to find out that the son, Murray Marks (Matthew Perry), is an air traffic controller at a private airport nearby.

When the big moment of Murray seeing his dad for the first time happens, it goes completely opposite of what J.D. expected. Murray tells his dad that he can have his kidney...for $70,000.

Murray later reveals that his dad has always done something to humiliate him, starting with naming him Murray. J.D. finally convinces him to do it.

But when the lab tests come back, they tell two different things...that Murray is a tissue match for Harrison, but Harrison is not Murray's father. As expected, Murray backs out of giving up his kidney. J.D. tries to reason with Murray. Dr. Cox, even though he warned J.D. about getting involved in patients' private lives, manages to talk to Harrison. Harrison says that Murray's mom was already pregnant when he met her and his dad skipped out, so they figured they just wouldn't tell him what really happened. Murray returns and tells his dad that he can have his kidney.

Meanwhile, Elliot gets coached from Jordan and Carla on how to use her feminine wiles to get more things at the hospital. Her flirting comes back to haunt her when she cuts in line and flashes some cleavage to get an orange soda. The person she cut in front of just happened to be the person interviewing her for a fellowship position. Carla hatches up an elaborate scheme to throw the interviewer off Elliot's track, but in the end, it's the janitor who makes him reconsider. All he did was to ask him if he wanted an orange soda, like the one he was denied while standing in line, and he threatened him with telling his wife that he is having an affair. It turns out the Janitor can imitate his voice extremely well.
